Form 9 - Claim for Sickness/ Temporary Disablement Benefit/Maternity Benefit

To claim sickness, temporary disablement, or maternity benefit, employee shall submit this Claim Form to the concerned branch office.

Any person who makes a false statement or misrepresentation for the purpose of obtaining benefit whether for self/another person shall be punishable with imprisonment of up to six months or with a fine of up to Rs. 2,000/-, or both.
